We aren't -- it's absolutely possible to write a story where history is radically different. But I have absolutely no problem with stories like "Civil War with all the exact same actors, but the North has airships." That is no less plausible, entertaining or "correct" than the other option.
I believe you, but it's sort of strange to expect the TV show to tell the girls in the TV show about birth control. Who would give that message? The cameraman? And for the record, Dr. Drew makes sure to grill them about contraception during the recap.

It would be nice if MTV forced (or strongly encouraged) the girls to attend teen mom support groups, but I don't really think it should be MTV's responsibility to act like one.

It's probably worth noting that there is a film crew in the room while she's telling him. If I had to guess, I'd say that a lot of these "revelation" scenes actually occur earlier, and are redone for the cameras.

I disagree pretty strongly with this one, actually. Yes, the figures should probably be *somewhat* different (Hitler won't hate Jews if Judaisim died out in the 3rd Century CE). But a lot of authors successfully play with the idea of historical inevitability.
